Appointments are required for physicals. The Ohio High School Athletic Association physical form that is used by participants is provided below for download. Please complete the form and bring it with you when you come for your physical. If health conditions are identified that might affect the athlete's ability to safely participate in sports, athletes will be referred for further evaluation.
