Sycamore Primary Care Center is open Monday through Thursday 8:30 a.m. to 5:00 p.m. Friday hours are from 8:30 a.m. to 1:30 p.m. Our address is 2115 Leiter Road, Miamisburg, Ohio, and we are located on the corner of SR725 and Gebhart Church Road (across from the Chevrolet dealership). The Sycamore Primary Care Center parking lot can be accessed from Gebhart Church Road or Leiter Road. Office hours are subject to change without notice. Once you are enrolled with Sycamore Primary Care Center, your health care will primarily be provided by one physician. If you have an urgent problem that requires care at a time when your doctor is unavailable, one of the partners who has access to all your medical records will provide your care until your primary physician is available.

After Hours / Emergencies
A Sycamore Primary Care Center physician is available after hours by calling (937) 866-0551. If you are a member of the Dayton Area Health Plan, please call (937) 222-3247 for assistance after office hours. In a medical emergency, please call 911 for immediate assistance.

Payment for Services
For your convenience, our office staff will complete and submit Medicare, FHP and private insurance forms for medical services you receive at our facility. Fees not covered by insurance are your responsibility.

Payment for service is preferred at the time of your visit. Visa, Discover and Mastercard are accepted. A statement will be mailed to you if you are unable to pay on the day of your office visit. Special arrangements are available for overdue accounts or financial difficulties. Please let the receptionist know if you need to speak with someone regarding your bill.
